Dave plays parts of the intro as notes together, and other parts as one note at a time. It depends.
The x's mean that you mute that string and nothing ("-") just means that you don't play that string.

Not necessarily. Unless there's a number on the string it's not played. It's either muted (which is usually, but not always, denoted) or not played at all.

I just mute the D string by letting my index and pinky fingers (for the hammer-ons) hang over the A string enough to mute the D string, just in case you accidentally hit the D string.
