Rather than picking two or more individual notes you slur them together by plucking the string only once.

mmm....not really. Maybe you explained it kind of confusingly. A hammer is where you play one note on a certain string, and use another finger to "hammer" on a higher fret, so that note will play also. So you only actually pluck with your right hand once, but you make two notes. So "5h7 on the B string" means you pluck the 5th fret (say your first finger is on it) and then hammer with your third finger on the 7th fret.
